Athletic Physical - Required Forms
All athletes must have a physical, submit the Consent for Treatment form, submit a copy of birth certificate, and have an unweighted GPA of 2.0 or higher.

The FHSAA requires all student-athletes for all sports at all levels to complete the free courses listed below prior to the first practice of their sport annually.
